Vision Coverage FAQ

Is laser vision correction covered by a vision plan?
It is important to be familiar with the provisions of your vision plan.  Laser vision correction is considered an elective procedure, and there may or may not be a benefit provided. Some plans provide a discount on the procedure as long as a network provider is used.  If your group is covered by a vision plan administered by Ochs, Inc., contact us for details.

What happens if I use an out-of-network provider?
Your cost-sharing is usually higher when you use an out-of-network provider.  The vision exam may be subject to a copay and frames and lenses may have additional material copays that apply.  In addition, members who elect to use an out-of-network provider may be required to pay the provider in full at the time of service and submit a claim for reimbursement.
